what is the difference between Immersion and Break 4th wall? I mean, they both went through Reality-Fiction right? please explain specific

Immersion and Break the 4th wall

One is going deeper the other is reaching out. Character A hopping into a fictional world via a book is immersion. But there is nothing for them to hop into so that they can acknowledge our existence as the reader/watcher. However 4th Wall Breakers know they are in a fictional world without hopping into some book within their fictional world. They can directly address us and know that everything even themselves is a character.
So Immersion goes deeper in and 4th Wall Breaking goes farther out.

does it allow 3D characters to Immersion into 5D Realm?

Depends if they are considered other worlds/universes or pocket dimensions or if they really are just fictional worlds that you can tear apart. If it's the first then yes if it's the latter then it's not really a 5D realm.
