Problem with your logic; the machine is literally a button. Pressing a button mid-combat isn't hard... it's a button.

Again, there's NO showing that 21 can't simply PRESS A BUTTON at the beginning of her battles. And again, NO. She never did it mid-combat because SHE NEVER NEEDED TO. It was always on. Always. No reason to turn it on again. You're basically saying "Because she never used it in combat, she can't press a button to turn it on again if it's ever off!" which is, frankly, terrible reasoning.

Vice shout is basically breaking down the local dimensional barriers. Any BFR within a local space can be bypassed by it, but it's never been shown entering alternate universes before.
