Itachi can create a 4-D Universe inside one's head and control the space and time.
His Mindhax isn't 4-D, everything that happens inside a brain isn't real, be it a multiverse, universe or whatever, it's all illusion no matter how big it is.

Why not? That would be 4D mindhax. Its not merely an illusion, its literally creating a mental construct the size of the universe. If a spiritual being is the size of the universe it would be universal durability right?

MindHax is 4-D when you affect a 4-D being of the real world, not when you create Illusionary Universe. It's true that this kind of mind hax is better then your average mindhax but VSB rules for mindhax are just outright bad.
By VSB standards someone who uses a basic mindhax on two people is better than someone who can create Illusionary Universe inside one human's head and control the space and time.
Rules favor numbers over quality.
