They slap each other around and don't achieve anything worthwhile cause they're both just dreaming, I think. Although Madotsuki is one dream layer higher if she's the Dream World version she stomps

Omori's killed the personalities of dreamers before, and he can also BFR Madotsuki to Black Space without killing her.

Considering Black Space is basically just what Madotsuki deals with normally, she probably wouldn't wake up immediately.
 
She definitely would within a week, in fact, that is already her reaction to BFR
 
It's also worth noting that Omori probably can survive the dream world being destroyed by going to White Space, since he does that more than once in his own story, so this might just be inconclusive through mutual BFR.

Maybe, but she also wouldn't really know to take advantage of this fact, since IIRC she herself dosn't know that she's still dreaming when she wakes up.
 
We usually use the latest version of the character, who in this case does. Nevermind that she eventually would figure it out if she does not already. (No dialogue makes it ambiguous)

It's also not like Madosuki is some kind of higher dimensional goddess who can kill Omori while outside of the dream world.

The most likely sequence of events is that Omori continues existing in White Space while Madotsuki wakes up. Madotsuki has literally no way of knowing about Sunny so she probably wouldn't wake up a second time.

Madotsuki literally would just sit in her room. She's more likely to go back into the dream world than to wake up a second time.

Omori also has the means to prevent Madotsuki from waking up, preventing her from pinching her cheek with Red Hands.

Omori also could erase Madotsuki's personality like he does with Sunny in some endings. Even multiple layers deep, Omori erasing Madotsuki's personality would probably affect her all the way up.

Omori can "do" that because he is part of Sunny's personality, the battle is symbolic.

No evidence of this.
