Well, Luffy doesn't start off with Gear 2nd, but as soon as he realizes his enemy is pretty tough he will use it.

Depends on Luffy's mood. He G2nd himself to one-shot the Pacifista. That was the very first thing he did post-ts. He also used it against the Hody Pirates and even to prevent Hody from cutting Neptune's head.
So yes. He can use it right off the bat and KR shouldn't work on Luffy if it works depending on the target's sins. Luffy's sin would be gluttony ovo.

Litirally everything he uses pasisvly poisons your soul, while also stacking damage on it (Ingame he simply ignores your invincibility frames and does 1 HP damage per frame.)
